User Name:Use your 5-digit member number (e.g., 1XXXX)

Spouse:Use the 5-digit member number followed by a capital S (e.g., 1XXXXS)

Password:Enter your last name in all lowercase (e.g., smith)

After logging in, you’ll be prompted to create a new password
